Confinement in a hospital: We will Indemnify under
Plan II $140.00 and Plan I $70.00 for the first 10 days,
Plan II $120.00 and Plan I $60.00 as of the 11th day and after.
Miscellaneous Expenses: Those incurred for the use of Operating Room,
Recovery Room, Laboratories, Material and Equipment,
Plan II $1,100 and Plan I $550.00.
Indemnity for Loss of Income: While hospitalized
Plan II $240.00 Plan I $120.00 per month.
Private Doctor: Plan II Maximum $40.00 Plan I $20.00 daily.
Nursing Services: Plan II will pay $60.00 per day, Plan I $30.00.
Benefits for X-ray, Radiotherapy, Cobalt and Chemotherapy services:
Plan II up to $6,000.00 per year. Plan I $3,000.00.
Indemnity for Blood and Plasma: Will covered the reasonable and
customary (includes Leukemia).

In case of hospitalization, you have the option of irrevocably converting
your policy and receiving the following substitution of basic benefits.
For the first 30 days of hospital confinement:

Plan I $ 66.00 daily or $2,000.00 monthly.
For the first 60 days of hospital confinement:
Plan II $ 100.00 daily or $3,000.00 monthly.

From day 91 and more:
